Each of our services includes a dedicated team of in-home nurses or aides, an off-site clinical manager available 24/7, and a scheduling coordinator.
Home Health Aide
Our skilled home health aides provide assistance to clients with mobility issues and early or late-stage dementia. Home health aides are supervised by a clinical manager.
An aide can provide assistance to clients in several areas, including:
Walking and transfers to help prevent falls
Grooming and dressing
Medication reminders
Light meal preparation and light housekeeping
Accompanying clients to doctor's appointments
Providing companionship
Encouraging approved exercise as instructed by physician
Monitoring and reporting any changes in behavior or health as instructed by the clinical manager.
Private Duty Nurse
Private duty nurses provide clinical monitoring to patients who may experience unexpected changes in their health status. Early intervention by a nurse can prevent serious complications or hospitalization. Private duty nurses are supervised by a clinical manager and Lorna McDonald, RN, BSN.

Communicating with doctors and designated family members to ensure the best possible care for the patient.
